I've just discovered that this exists. It is an interactive map showing HIV prevalence by region in the US. Here's a sample:

Prevalence of HIV diagnoses, rate per 100,000

Data comes from the CDC. A caveat I'd add, if I'm reading their notes right, is that they are only using the data on prevalence of diagnoses, rather than estimates of the actual prevalence of HIV itself. Not everyone who has HIV has been diagnosed yet, and depending on what you intend to do with the data, that can be a very important distinction.
